Please download, print, fill out, and bring with you to your first massage appointment a Health History Form. You will need Adobe Acrobat Reader -- click here if you do not have this free program and wish to download it.

Bodywork is not a substitute for medical advice. I do not diagnose, prescribe medications or herbs, or give advice to clients regarding their medical condition.

My work is strictly non-sexual. Inappropriate client behavior will result in immediate termination of the session without refund of session fee.

Early or late arrivals:
Please refrain from arriving more than 10 minutes early (unless it is your first appointment & you need to fill out a Health History form) so that I can be ready for you.

If you are late, your massage session will be shortened by that amount of time; in other words, your session will still end at the time it was scheduled to end.

Cancellations:
If you cancel with less than 24 hours’ notice or do not show for your massage appointment, unless due to illness (including active colds, etc.) or emergency, payment for the session is due in full.

Inclement Weather:
If Montgomery County schools are delayed or closed, you or I have the option of cancelling your appointment. This constitutes an emergency cancellation and you will not be charged for your appointment, unless you simply do not show up without any notice. Cancellations should be made at least 2 hours in advance in the case of inclement weather.

No Tipping:
I am a health care practitioner and my rates reflect what I feel to be a fair value for my services. No additional compensation is required or expected.
